Which nephrology patients should receive parenteral iron?

I believe that virtually all nephrology patients receiving erythropoietic therapy, either Epogen® or darbepoetin alfa, are now supplemented with i.v. iron to maintain the TSAT greater than 30% or the ferritin up to 800 ng/ml. The KDOQI are about to redo their guidelines, but right now virtually 100% of dialysis patients and most predialysis patients receiving erythropoietic therapy are getting parenteral iron, says Dr. Michael Auerbach, Clinical Professor of Medicine, Georgetown University School of Medicine.
